Vendor Registration Form
Before a vendor is eligible to sell goods and/or services to the University of North Carolina School of the Arts (¶¶Òõ¶ÌÊÓÆµ), a Vendor Registration Form is required. Vendors MUST submit a Federal W-9 along with this form; however, it cannot be submitted in lieu of this form. Foreign Nationals are also required to submit a Foreign National Information Form along with the Vendor Registration Form.
Whenever a change occurs in the information submitted, such change will be reported immediately using the same form.
All vendors are also encouraged to register with the State of North Carolina as a vendor, which is separate from this registration. To do so, please visit the .
